A prospective longitudinal study documenting the incidence of infantile hemangiomas and their association with placental anomalies

The etiology and incidence of infantile hemangiomas (IH) are unknown. Prior studies have identified immunohistochemical and biologic characteristics shared by IH and placental tissue. We sought to identify the possible association between placental anomalies and the development of IH, as well as other risk factors associated with the development of IH. A prospective longitudinal study enrolled 578 women in labor who gave birth to 594 infants. All infants were followed for at least 9 months. Thirty four hemangioma-type lesions were identified in 29 infants, 2 of which were proved to be rapidly involuting childhood hemangimas (RICH). Thirty five percent of hemangioma -associated pregnancies were associated with placental anomalies, defined by either abnormalities of the cord/placenta or a history of placenta previa or abruption. In contrast, only 17% of pregnancies leading to non-affected infants were associated with placental anomalies, a statistically significant difference (p = 0.025). When only classic IH associated pregnancies were compared to non-affected pregnancies, the statistical significance was even greater (p=0.013) Other risk factors for IH included prematurity, low birth weight, Caucasian race, and female gender. All IH were present by 3 months of age, and all had ceased growth by 9 months. The majority of hemangiomas occurred on the trunk, in contrast to previous reports in which head and neck locales predominated. Of note, 20% of identified IH were abortive/telangectatic in nature, focal lesions that did not proliferate beyond 3 months of age
