Comparing US to EMG studies in screening for CTS in the paraplegic population

Carpal Tunnel Syndrome (CTS) is traditionally diagnosed by electrodiagnostic testing, but ultrasound (US) has also been studied as an alternative method of diagnosis in able -bodied individuals. This study compares US findings, specifically cross-sectional area (CSA), to electrodiagnostic data and reported symptoms for comparison in screening for CTS within the paraplegic population. Seven patients with paraplegia underwent US and electrodiagnostic testing in addition to reporting symptoms using the Levine questionnaire. US correctly diagnosed CTS in all 6 subjects who had nerve conduction parameters consistent with CTS. We found significant correlations between the CSA at the level of the pisiform to the electrodiagnostic measure of sensory latency (p=0.04, r²= 0.39) and distal wrist crease to sensory (p=0.02, r²= 0.48) and motor latency (p=0.02, r²= 0.36). There was also significant correlation between the CSA at the distal wrist crease to symptom, function and total score of reported symptoms, whereas pisiform CSA was nearly significant only in predicting total score. From our results, we concluded that US appears to be an acceptable alternative to electrodiagnostic testing in diagnosing CTS within the paraplegic population although additional prospective investigation is needed
