Assessing asthma management practices to identify barriers in provider education : a comparison of NHLBI guideline compliance among inner-city pediatric primary care providers in San Diego, CA and the Bronx, NY

Following a 2006 study noting that physician compliance with the NHLBI guidelines in the South Bronx needed improvement, particularly among primary care providers (PCPs) in private practice, a re-examination was expanded to other areas of the Bronx and to San Diego to analyze the awareness of the 2007 Expert Panel Report 3 (EPR-3). A 10-point questionnaire highlighting specific areas of focus in EPR-3 was administered to 52 community-based, 49 hospital-based PCPs and 33 residents practicing in inner- city neighborhoods in the Bronx, and to 36 community-based, 12 hospital-based PCPs and 8 residents practicing in urban areas of San Diego. Comparison of total scores between community-based with hospital-based PCPs and residents in the Bronx showed a trend (p=0.06) towards lower rates of compliance among community-based practitioners, particularly for prescribing a spacer with an inhaler (p=0.0004). Preferred prescription of pills over inhalers, and low rates of referral to community health professionals for asthma self-management education were observed in all three groups in this area. In San Diego, residents demonstrated a lower overall compliance (p=.004), particularly for preferred prescription of pills over inhalers and acknowledgement of ethnocultural differences in treatment of asthma. Low rates of referral to community health professionals for asthma self-management were observed in all groups; however, the effect was most prominent in the residents in comparison to private practitioners
